If you or a loved one lives in qualified senior or subsidized housing and needs assistance with daily personal care, the Group Adult Foster Care (GAFC) program may provide the support necessary to remain safely at home.

What is GAFC?

Group Adult Foster Care (GAFC) is a MassHealth program designed to serve eligible individuals living in qualified housing who are elderly and/or disabled.

The program provides assistance with activities of daily living (ADLs) and support services based on individual eligibility and care plan needs, helping individuals continue living safely in their homes and community.

Eligibility Requirements

You may qualify for GAFC if you:

  • Are 22 years of age or older.
  • Have MassHealth Standard, Senior Care Options (SCO), or One Care coverage.
  • Require daily assistance with personal care.
  • Meet clinical eligibility requirements as determined through assessment and MassHealth guidelines.
  • Live in qualified subsidized or senior housing approved for GAFC services.

Activities of Daily Living (ADLs)

Assistance with activities of daily living for eligible individuals may include:

  • Personal care
  • Bathing
  • Dressing
  • Grooming

Instrumental Activities of Daily Living (IADLs)

Support related to instrumental activities of daily living, as appropriate and based on an individualized care plan, may include:

  • Meal preparation support
  • Medication cueing and reminders
  • Safety monitoring and supervision
  • Limited assistance related to daily household tasks, as outlined in the care plan

Services are provided in accordance with MassHealth guidelines and individual eligibility requirements.

What Our Team Does

GAFC participants receive care coordination and clinical oversight from experienced professionals:

  • Registered Nurse (RN): Conducts scheduled wellness visits and health monitoring
  • Case Manager: Coordinates services, reviews care plans, and supports individual care needs
  • On-Call Clinical Support: Available for urgent care-related concerns in accordance with program guidelines

We work collaboratively with housing staff and family members to promote safe, respectful, and person-centered care.
