If you or a loved one needs daily personal care and supervision at home, the Adult Foster Care (AFC) program may provide support while allowing individuals to remain in a familiar and safe environment.
What is AFC?
Funded by MassHealth (Medicaid), Adult Foster Care allows a family member or trusted individual to serve as a caregiver for eligible members in their homes. Caregivers provide daily care and supervision based on an approved care plan.
Individuals do not need to be bedridden to qualify. The program is designed for those who require supervision or physical assistance with activities such as mobility, transferring, dressing, toileting, eating, and bathing. Eligible caregivers may receive a monthly tax-free stipend for providing ongoing personal care and support.
Eligibility Requirements
You may qualify for AFC if you:
- Are 16 years and older.
- Have MassHealth Standard or CommonHealth for medical insurance.
- Require assistance with ADLs and IADLs daily.
- Have a medical condition or cognitive impairment that requires hands-on assistance or active supervision and coaching to complete ADLs.
- Require ongoing supervision and support as determined through clinical assessment and RN evaluation.
